Your household matters to us!!
Please make sure to list all students in your household who are currently enrolled in
Mineral Wells Independent School District on the Free and Reduced Lunch Application.
Each eligible student that is enrolled in this program will ultimately benefit our entire district.
If you have MWISD students in your household now that were not listed on the initial application
that you submitted at the beginning of the school year, please contact us by phone (940.327.5102)
or email: kellyb@mwisd.net to make the appropriate changes.
As you begin receiving your 2010-2011 School Breakfast/Lunch Application, please be sure to
include the students' first and last name, date of birth, and grade in the top section of the form. A parent must sign and date the application, as well as provide a Social Security number
for the household if one is available.
Applications must be complete in order to be processed and eligibility determined.
